(Christchurch) Social Worker - Primary Health Organisations (PHO)

Based in Christchurch:

  • Part time fixed term position: 24 hrs per week as a Social Worker in our Community Family Works Team  
  • Fixed term Contract till 30th June 2025 
  • Based in central Christchurch office of PSUSI and mobile in Medical Centres

Ngā mōhiohio e pā ana ke tēnei mahi (About the position)

The key purpose of this Social Worker role is to work collaboratively to provide place-based high quality, professional and time-framed social work services for adults, parents, children, young people and families enrolled with Christchurch PHO general practices in Christchurch which support and encourage and enhance positive outcomes for individuals and family or other relationships. 

This may include co-working with other Presbyterian Support Staff or other agency staff. To build strong positive relationships with GP Practice staff who act as referral agents. Ensure appropriate engagement with other support networks and agencies in Christchurch that will enhance outcomes for clients.

This position supports the delivering of the Social Work initiatives within our Community Family Works team which has a particular focus on working to advocate and navigate on behalf of our clients with the medical practices and the wider community.

Ko wai he kaitono tōtika? (Who is an ideal applicant?)

Our ideal Candidate will:

  • Have 2-3 years’ experience as a registered Social Worker 
  • Able to provide effective individualised support, advocacy and navigations skills on behalf of our clients and their families/whānau. 
  • Have experience and the confidence to develop strong relationships with stakeholder groups e.g. Medical Centre staff and health providers, other key community agencies quickly and effectively
  • Passionate about supporting adults, children, young people and parents
  • Able to take responsibility, show initiative and is punctual and self-motivated
  • Enjoys being a part of a team but is able to work independently in a fast-paced mobile environment
  • Highly organised with the ability to prioritise and meet expected deadlines
  • Able to provide effective individualised support, advocacy, interventions by navigating effective solutions for clients and their families/whānau. 
  • Demonstrated ability to respect and adheres to confidentiality and professional boundaries 
  • Hold knowledge of relevant community networks and resources including those important to Maori and Pacifika families 
  • Sound computer skills including the use of email, internet and Microsoft applications (Word/Excel/PowerPoint) and the ability to learn new client management systems quickly
  • Capture and maintain accurate and timely client information using the relevant database. This includes maintaining client details, assessment information, evaluations, goal plans, and activity management 
  • Ensure progress and statistical data is recorded regularly, accurately and consistently 
  • Work effectively in a team environment and have the ability and experience to support the Team leader and the wider Family Works team 
  • Have a high degree of professional integrity and honesty, able to manage sensitive information confidentially.
  • Holds a full clean driver’s license
